CASE MANAGEMENT

At Westminster Free Clinic our case manager meets with each patient at the clinic and connects them with county services and helps with applications related to Medi-CAL, Medicare, and other programs when a person is eligible. The case manager also links our patients to community partners and helps them communicate with medical specialists when these services are needed. The case manager also creates partnerships with optometrists, dentists, and others to support the local underserved community.

STATS

Our Case Manager, Marlene Ramirez, CNA, meets with every patient each clinic night to make sure they understand their plan of care. She also coordinates our mammogram events in which 200+ women are screened for breast cancer at WFC each year. She also provides colorectal cancer screening tests to eligible patients on a weekly basis.
