High school interns, and other volunteers work in partnership with the bi-lingual health educator and dietitian to implement and offer the program every week. The teen volunteers help and also learn to improve their own health and the health of their family.
Heart disease, also called cardiovascular disease is the top diagnosis at WFC, but with prevention and medical care, patients can take control of their condition to be able to continue to work, care for their families, and have a long, healthy life. The Corazones Sanos (Healthy Hearts) program is culturally sensitive, and offers services that consider the many barriers, including low-literacy, that patients face.
In 2025, cardiovascular disease, remains the leading cause of death in the United States. About 2,500 Americans die of cardiovascular disease each day and about 126.9 million American adults are living with some form of cardiovascular disease. However, a high percentage of heart disease risk factors can be modified. By adopting a healthy lifestyle, we can take steps to preventing costly heart problems, and keeping people well and working.

95% of all the diabetics in the United States have Type 2 diabetes and by 2050, 1 in 3 people may have diabetes. In addition to being one of the fastest growing chronic diseases and top 10 causes of death, it is also very costly to our society when left untreated. In 2022, $412.9 billion was the total estimated cost of diagnosed diabetes in the United States.

The Corazones Sanos (Healthy Heart) program uses culturally sensitive, participant-centered services such as:
- Early detection of high blood sugar and blood pressure at weekly community health screenings and at our clinics
- Early access to preventative health, dental, and vision care
- Nutritional education
- Experiential learning and life enriching services offered
- One-on-one and group social emotional support programs
- Empowerment programs and activities
- Food as Medicine/ food distribution program with free fruits and vegetables and other heart healthy food offered weekly at WFC to over 600 families
- Leadership experiences available for participants
- Job skills training for teen program interns
- Free diabetic home test kits and home blood pressure machines given out to patients to monitor their health and improve their health
